question:I am 43 year old female and have a bicuspid aortic valve. I work full time, but I am so tired at the end of the day I have to rest when I get home. I feel dizzy at times, out of breath, I have stomach angina and the joints in my fingers are achy. Can I receive disability for this?
answer:Hi Ma'amIts a pleasure to answer to your query, as you have mentioned that you have a bicuspid aortic valve and you feel tired at the end of the day. Now let me explain the mechanism happening in your case, aortic valve which is generally tricuspid is bicuspid in your case which means the opening will be less in your case due to which less amount of blood passes through the aorta when your LV pumps due to this less amount of blood is being supplied to your body. Due to this when you are physically active a SUPPLY DEMAND mismatch happens due to which you feel tired and dizzy at the end of the day.It can be take care of with the help of meds upto certain extent but in case if you develop symptoms like giddiness or palpitations then it need to be corrected by means of surgery. Now regarding the stomach pain usually cardiac pain does not refer below diaphragm so the pain which you are feeling is probably non cardiac and for that you need to check with your gastroenterologist. And also get one Serum Uric acid and ASO titre done for your join pain.Good Luck

question:YYYY@YYYY My husband has been diagnosed with IPF and is currently suffering through coughing attacks that he believes are related to PND. He has tried the over the counter allergy meds on advice from his pulmonary doctors but nothing seems to help. He has finally agreed to see a physician but I am not sure whether to schedule with an allergist or an EENT. He recently has suffered from dizzy spells and/or blurred vision that lasts only a few minutes when he turns his head and then turns back quickly which I thought could be related to his inner ear. He does not complain of pressure in his ears or sinuses. Do you have any suggestions about the medication for PND or to help his coughing subside? He constantly uses cough suppressants. Thank you for any advice you have.
answer:Hello, As you explain the situation he should see a pulmonologist specialist at least and then he or she will decide if it is from PND and what to do further. Hope I have answered your query. Let me know if I can assist you further. Regards, Dr. Jnikolla, Pulmonologist
question:I was diagnosed with IPF two years ago. My only symptom before I went to see the doctor was persistent cough dry cough. Since the past 7 days I now have been experiencing frequent hacking cough. Saw my family doctor 2 days ago and got prescription cough syrup and antibiotics. Am worried that this could be a result of the progression of my disease. The cough syrup and antibiotic have not helped much but I have phlegm early in the mornings since I started taking my meds.
answer:Hello, According to history, you should be followed up by a specialist of IPF. Please talk to your doctor to send you to the specialist. Hope I have answered your query. Let me know if I can assist you further. Regards, Dr. Jnikolla, Pulmonologist
question:I was diagnosed with pluerisy about 2 weeks ago. It seems to have cleared up. I am still running a fevor. It has been at least 2 weeks. I have been laying low the whole two weeks. It has been a steady 99-101. I went back to my doctor and he said they dont worry about fevers until it has been going on for 5 or more weeks. He thinks i caught a virous on top of what i orginally had, but does a virus go on for this long? I think that is completely rediculous. Please help. What is wrong with me?
answer:Hello, As you explain the situation yes I agree with your treating doctor. Do not worry about. It might be a viral situation actually. Wait and see. Hope I have answered your query. Let me know if I can assist you further. Regards, Dr. Jnikolla, Pulmonologist
question:3 wks ago I was diagnosed with pneumonia via x-ray & treated with antibiotics. Felt better after a week then 3 days after antibiotics finished my pneumonia came back and worse, confirmed by x-ray. I just finished my 2nd course of antibiotics and have generally been feeling better. About 4 days ago (just before finishing antibiotics) I developed extreme pain in my chest and back as if I have been stabbed through directly below my shoulder blade. I still have a mild cough and occasionally bring up clear phlegm and have no fever. I am scheduled to take a follow up x-ray next week, but I am concerned about this pain. Could it just me muscle pain from 3wks of coughing, could the pneumonia be returning, or could there be other complications?
answer:Hello, Usually, the recovery phase of pneumonia can also give rise to stretching pain from underlying structures of the lung parenchyma. Rather than getting panic of recurrence, just wait for the follow-up x-ray for further guidelines, till that time, I would recommend to - do regular deep breathing exercises - walking in fresh air - maintenance of a balanced nutritious diet to combat any underlying pathogen with the best immunity - avoid exposure to noxious substances as pollen, house dust etc. - avoid smoking if involved. Hope I have answered your query. Let me know if I can assist you further. Regards, Dr. Bhagyesh V. Patel, General Surgeon
